COLE MEMORIAL HOSPITAL
was
recently honored with another Hos-
pital & Healthsystem Association of
Pennsylvania (HAP) Achievement
Award. Cole Memorial was recog-
nized by HAP in 2011 for its com-
munity advisory board program.
In 2012, HAP gave an operational
excellence award to Cole Memorial for the emergency
department’s improvement in patient satisfaction and
a patient care award to the hospital’s community care
transitions team.
COLE MEMORIAL RECEIVES FOURTH HAP AWARD
According to HAP, the HAP Achievement Awards
offer a way to recognize outstanding efforts, such as
discovering ways to engage the community in wellness
initiatives, improve technologies to better serve patients
and reduce costs, and enhance the way patients are cared
for and treated.
Under the patient care category, Cole Memorial was
recognized for Reducing Frequent Emergency Depart-
ment Visits through Coordinated Care.
Among the project’s team members were Marlene
Wust-Smith, MD, FAAP; Janice Walters; Kristin Kratzer;
Jackie Plummer-Sprow; Donna Farrell; Lori Gross; Sue
Pascarella; B.J. Lowther; Tracy Leet; Samantha Wetzel;
Eunice Hamby; Brigitte Green; and Rob Leete.
“The HAP Achievement Awards are worth more than
a simple round of applause,” says Andy Carter, HAP’s
president and CEO. “They represent the proactiveness
of hospitals who are taking big steps toward a healthier
Pennsylvania, which affects every single one of us in
the state.”
